Helping the NHS – Test Track Trace

RCCG Rabboni Parish is taking great steps to welcome everyone back safely into the church premises from Sunday 4th October 2020. With Covid 19 cases on the rise, it is essential we play our part in supporting the NHS to control the spread of the virus.
We can play our part by supporting the NHS to trace anyone who has had close contact with anyone who has tested positive for coronavirus. Where this happens and where necessary we will be able to notify any affected persons to self-isolate to help prevent the spread of the virus.
Our purpose is to provide a safe environment when worshipping at the church premises. it is also essential that when coming into the premises we all follow the safety guidelines of Face, Hands Space
· Wear a face covering
· Use hand sanitisers
· Maintain social distancing (2m or 1m+)

To keep our church premises open we are using the options below to collect and maintain the records of all visitors coming into our church premises.

1. Complete and submit an online form 24hrs in advance
2. Scan the church QR code into your phone

We are prioritising option 1 to encourage everyone attending church on a Sunday to give prior notice of the advance of their attendance. Individuals from the same household must nominate a lead person to enter their contact details and the number of people attending where applicable. However, Individuals attending church as a group from different households are advised to enter each person’s full name and phone numbers on the form. Anyone who does not provide details in advance, at least 24 hours before attending will not gain automatic entry until prior bookings are seated, the church QR code is scanned and the form is completed and submitted.
The links to the contact form will include the following details.
· Full name
· Mobile number
· Date of Visit
· Number of people in the group

To align with the GDPR rules you are required to give your consent by ticking the box below on the contact tracing form. This means that you are consenting for your contact details is used for this purpose only.
We strongly advise that you remember to tick the consent box before submitting the form.
Option 2 refers to the QR code to scan using your mobile phone. This QR code is for use where visitors have not booked online prior to attending church. Everyone entering the church premises, in this case, is advised to scan the QR code using their mobile phone which allows you to complete and submit your details.

Please note we all must play our part
This means that:
· if you develop symptoms, you must continue to follow the rules to self-isolate with other members of your household and get a test to find out if you have coronavirus
· if you test positive for coronavirus, you must share information promptly about your recent contacts through the NHS Test and Trace service. This would help the NHS to alert other people who may need to self-isolate
· if you have had close recent contact with someone who has coronavirus, you must self-isolate if the NHS Test and Trace service advises you to do so
· if you are returning from travel abroad it is important to check whether you need to self-isolate
